• Bring this Vaccine card with you to the designated immunization center on the specified date for taking 1st and 2nd dose of Covid-19 Vaccine.
• Inform the Immunization Officer immediately if there is any problem / difficulty after vaccination. If necessary, bring the intended people to the nearest health center.
• Before the vaccination, the vaccination center and the date of delivery will be informed through SMS.
• Save the card for future use even after the vaccination is over.
• The Vaccine card can be downloaded from the https://surokkha.gov.bd website even if it is lost. Certificate can be collected from https://surokkha.gov.bd after completion of 2 doses of covid-19 Ticker.
• Even if you get vaccinated against covid-19, follow proper hygiene rules.
